- When the patient is at the second degree, he can remain seated and standing and walk with assistance or using crutches. He can move his arms and be self-sufficient.

- When the patient is at the third degree, he can remain seated but is incapable of standing. He can use his hands, but with certain difficulties, and he is not self-sufficient.

- And at the fourth degree, the patient cannot walk or remain seated. He can remain lying down, and the muscles are very atrophied and do not work normally. It is the most serious degree, and it requires the help of other people.

This scheme helps us to determine the possible results of this treatment, and it facilitates an approach regarding the tactics of the appropriate treatment for this group of patients. For example, in some cases, the patient can gradually go from a high degree to a lower degree. In some exceptional cases, the patient can go from the third degree to the first degree. And when the patient is at the fourth degree, treatment may simply offer the family a decrease in difficulties for caring for and assisting the patient, given that passive movements are increased, thereby decreasing stiffness and in turn allowing the patient to rest and feel better.

My 10 years of experience developing this technique have shown me that the best way is to begin operating on patients as from 3 years of age, when there are still no dislocations, deformities or contractures in the joints and bones. A child must be given the opportunity to walk when the muscles are not quite so atrophied. With the passage of time, if a patient does not undergo any treatment or get any rehabilitation, the contractures become more rigid and numerous, and muscular atrophy increases.
